Care provided through Zepym Health is delivered by telehealth (secure messaging, video, and asynchronous review). Telehealth has benefits (convenience, access, privacy) and limitations (no in-person physical exam, potential technology failures, limited ability to manage urgent issues). By using the Service you acknowledge these benefits and limitations and consent to receive care by telehealth. You may withdraw consent at any time and seek in-person care.

Your Clinician may determine that telehealth is not appropriate for your situation and may refer you to in-person care. You are responsible for following any such referral.

6.GLP-1 Risks & Boxed Warning

Boxed Warning — GLP-1 Receptor Agonists

GLP-1 receptor agonists, including semaglutide and tirzepatide, carry a boxed warning regarding the risk of thyroid C-cell tumors, including medullary thyroid carcinoma (MTC). These medications are contraindicated in patients with a personal or family history of MTC or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Counsel patients regarding the potential risk of MTC and the symptoms of thyroid tumors (e.g., a mass in the neck, dysphagia, dyspnea, persistent hoarseness).

Additional risks may include pancreatitis, acute gallbladder disease, acute kidney injury, hypoglycemia (especially in patients on insulin or sulfonylureas), hypersensitivity reactions, diabetic retinopathy complications, and others. Discuss your full medical history with your Clinician before starting therapy.

7.Contraindications

You should not use GLP-1 receptor agonist therapy if any of the following applies, unless specifically directed by your Clinician with full knowledge of your history: personal or family history of medullary thyroid carcinoma; MEN 2 syndrome; prior serious hypersensitivity to a GLP-1 agonist; history of pancreatitis (relative contraindication); pregnancy, planning pregnancy, or breastfeeding; severe gastrointestinal disease including gastroparesis; or end-stage renal disease without specialist oversight.

8.Common Adverse Effects

Common adverse effects of GLP-1 therapy include nausea, vomiting, diarrhea, constipation, abdominal pain, decreased appetite, fatigue, headache, and injection-site reactions. Most effects are mild to moderate and improve with time and dose adjustment. Contact your Clinician promptly for severe or persistent symptoms, severe abdominal pain (which may indicate pancreatitis), or signs of dehydration.

9.Compounded Medications

In some cases your Clinician may prescribe a compounded medication prepared by a state-licensed compounding pharmacy operating under Section 503A of the Federal Food, Drug, and Cosmetic Act. Compounding may be appropriate when a commercially available product does not meet a specific clinical need. Your Clinician will discuss the risks and benefits of any compounded therapy with you.

10.Off-Label & Non-FDA-Approved Use

Certain medications offered through the Service may be prescribed for off-label use — a use, dosage, patient population, or route of administration that the U.S. Food and Drug Administration (FDA) has not specifically approved. GLP-1 receptor agonists such as semaglutide and tirzepatide, for example, may be prescribed off-label for weight management or other wellness goals, and the compounded formulations described above are not FDA-approved drug products. Off-label prescribing of lawfully marketed medications is a common and legal part of medical practice when supported by clinical evidence and the Clinician’s professional judgment. Off-label use may carry risks and benefits that are not reflected in FDA-approved labeling. Your Clinician will discuss any off-label recommendation with you before you begin treatment.

11.State Licensing & Scope of Practice

The Affiliated Medical Group’s Clinicians are licensed in specific U.S. states. The practice of medicine is governed by the laws of the state where you are located at the time of the encounter. If you relocate, notify us — we may not be able to continue your care in states where our Clinicians are not licensed.

12.State Medical Board Complaints

If you have a concern about the professional conduct or quality of care provided by a Clinician, you have the right to file a complaint with the medical board of the state in which you are located. Most states accept complaints online or by mail through their medical licensing board — for example, the Florida Board of Medicine (flboardofmedicine.gov), the Medical Board of California (mbc.ca.gov), the Texas Medical Board (tmb.state.tx.us), or the New York State Board for Professional Medical Conduct (health.ny.gov).
